Verification Forms

• Asset Verification Form
  Verify student and/or parent asset information.
• Dependent Household Size Form
  Parents of dependent students clarification of household size.
• FAFSA Signature Page
  Use this form if contacted from our office that your FAFSA was rejected by the federal processor because it was not signed.
• Federal Loan Discharged Due to Disability Form
  Complete this form if you have had a federal student loan discharged due to disability.
• Independent Household Size Form
  Students clarification of household size.
• Selective Service Form
  Students whose Selective Service registration cannot be verified must complete this form. Male students are also required to contact Selective Service.
• Verification Worksheet
  For independent and dependent students, please make sure the form is complete with all required signatures.
• Veteran Benefits Form
  Veterans use this form to report VA chapter benefits.

Reviews and Other Documents

• Consortium Agreement
  Use this form when you are attending two schools. You must make payment arrangements at the participating school or you risk being dropped for non-payment. Consortium Agreements are subject to approval and may not be granted if classes are not part of the students program of study on file, if the student has not maintained MCCCD academic progress, or if the student is not taking 6 or more credit hours at GateWay.
• Dependency Review Form
  Use this form to appeal your dependency classification.
• Request for Continued Funding Form
  Use this form to request a reevaluation of your financial aid funding based certain circumstances.
• Suspension Review Form
  Use this form to appeal Financial Aid Suspension status.
